This agreement is between Crissy's Camp Canine and you, the Client. You affirm that
you are the legal owner or authorized caretaker of the pet(s) listed.
Requested times are approximate and may vary due to weather, traffic, emergencies, and scheduling demands.
Have more than two pets? Note them in the care instructions below, or ask Crissy for a Pet Profile Form.
Please describe feeding schedules, water, exercise, medications, behavioral concerns,
household routines, and emergency procedures. The Company will rely on the instructions you provide.
You authorize the Company to seek veterinary treatment if it reasonably believes
emergency care is necessary, and agree to be financially responsible for all veterinary expenses.
As part of pet sitting, the Company may perform limited home care tasks. Check any you'd like:
Deposit is due at time of booking; final payment is due within 24 hours of the final
date of service. Crissy will confirm exact fees with you. Preferred payment method(s):
The Company may send you updates, photos, and videos of your pet(s) during service.
May we also use non-identifying photos for marketing?
Client representations: You certify that all information provided is accurate
and complete; your pets are current on legally required vaccinations and free from contagious
diseases unless disclosed in writing; and you have disclosed all behavioral concerns, including
any bite history, aggression, escape tendencies, or anxiety-related behaviors. Failure to disclose
relevant information may result in immediate termination of services.
Cancellation policy: Cancellations more than seven (7) days before service:
full refund. Cancellations less than seven (7) days before service may result in charges up to the
full reservation amount. Holiday bookings may require non-refundable deposits and extended notice.
Early return: If you return home earlier than planned, scheduled visits
already reserved may remain billable and non-refundable.
Pet health & safety: The Company may discontinue services if a pet becomes
aggressive or dangerous, unsafe conditions exist, or false information was provided.
Emergencies: In the event of pet illness, injury, escape, natural disaster,
utility failure, or home emergency, the Company is authorized to take reasonable actions to protect
the pet(s), property, and people involved; associated costs may be charged to the Client.
Limitation of liability & indemnification: The Company exercises reasonable
care, but animals may behave unpredictably and you accept inherent risks of pet care. To the fullest
extent permitted by law, the Company's liability is limited to the amount paid for the services.
You agree to indemnify and hold harmless the Company from claims arising from your pet's actions,
undisclosed behavioral or medical issues, inaccurate information, or unsafe property conditions.
Surveillance: You agree to disclose any interior/exterior cameras or recording
devices on the property.
Governing law: This Agreement is governed by the laws of the State of Ohio and,
together with any Pet Profile, Veterinary Authorization, Policies, and Addenda, constitutes the
entire agreement between the parties.
